ADVOKID — IEP PROGRESS-READING WORKSHEET https://www.advokid.ai/resources/iep-progress-reports Updated September 21, 2026. General education, not legal advice. Edit on your own device or print. AdvoKid does not receive what you add. Use one copy per goal. Keep source pages; leave missing information unknown. Goal wording and IEP version date: Starting point / baseline: Target and how it is measured: When the IEP says progress reports will be provided: District code / meaning from the report's own legend: REPEAT FOR EACH DATED UPDATE Reporting period / source page: Result exactly as reported (do not replace a missing result with zero): Task and unit (for example, correct responses out of opportunities): Assistance, prompts, setting, or task conditions: One observation, an average, or another summary?: Can this fairly be compared with the previous result? What changed?: What the school explained: Parent observation and date (keep separate from school measurement): Question / person to contact / agreed check-in: USEFUL QUESTION What are the dated results, what assistance was used, and how do these compare with the starting information and the goal's actual conditions? A label such as progressing is not a complete explanation. A single missed target, changed condition, or missing record does not prove a violation. Discuss uncertainty with the team. Seek qualified advice for a disagreement.
